Subject: [PATCH] publish: drop diff detection, always run --all

Git diff is unreliable as a publish gate — failed runs silently drop
files. Wiki idempotency (commit SHA in banner) is now the sole
gating mechanism: unchanged pages are skipped, changed pages update.
